Players are required to have photo ID at every game in order to be allowed to play.  Options for on-field photo ID include ONLY:

• Saskatchewan Governement Issued photo ID (ie. Driver's license)

• Saskatoon Adult Soccer Player ID Card

Players have 2 Options to purchase a Saskatoon Adult Soccer Player ID card:

OPTION 1 - In-Person: Visit us at the Saskatoon Adult Soccer offices during business hours.   

REQUIRED: 

1. Government Issued Photo ID  - any official Government ID.

2. $20 Cash OR E-transfer to info@saskatoonadultsoccer.com with Registered Player Name

*a headshot will be taken for your ID card in office

OPTION 2 - Online: Complete this Application Form below.

REQUIRED:

1. A picture of your Government Issued Photo ID  - any official Government ID.

2. $20 E-transfer to info@saskatoonadultsoccer.com with Registered Player Name

3. A .JPG image of the headshot you wish to be placed on your Player Card.

* To make a Player ID Card, we can review all official Government supplied photo ID but reserve the right to request a different form of ID.

Please expect photo ID cards a minimum of 3 - 5 business days to complete from having received the request.
